Getting a Health Card
A health card is an important thing for all residents of Ontario to have because it’s what you show when you visit a doctor or hospital. Most of our health card is paid for by the government, and this piece of photo identification, which you receive at birth, is your way of receiving this care. If your card is lost, stolen, or broken, the you must report it immediately. For health card services, including registration, renewal, or if you have changed your address, visit one of the places below.

Forms and Official Documents — birth, marriage and death certificates and registration searches can be found here. Forms are not submitted at this office, but can be done online or through the mail. A commissioner of oaths is always on staff
Business Registrations — basic business registration can be completed using the public access computer, though this can also be done online at home. Business Info Line 1-888-745-8888
Employment — filing of claims under Employment Standards Act, and Employment Protection for Foreign Nationals Act (Live-in Caregivers and Others). Claims are not processed at this office, simply filed.
Ontario Landlord and Tenant Board Applications can be found and filed at this locations, but are sent away for processing.

Generally, if application forms are filled correctly, turn around time is about 6-8 weeks to receive ‘name change certificate’.

- Investigate allegations or evidence that children who are under the age of sixteen years or are in the Society’s care or under its supervision may need protection

- Provide guidance, counselling and other services to families for protecting children or for the prevention of circumstances requiring the protection of children

The Huron-Perth Children’s Aid Society (H-PCAS) serves children, adolescents and families under the Child and Family Services Act
Services provided by the H-PCAS include:
- Child Protection
- Counselling services to children under 16 years of age and their parents
- Placement of children at risk in individual and group home foster family settings
- Adoption placement of infants and older children who are legally and emotionally free for adoption
- Supervision, finalization of adoptions through the court
- Ongoing assistance to adoptive families
- Information Dissemination regarding “Duty to Report” suspected cases of child abuse and neglect through the Purple Ribbon Campaign
